We describe a case of malignant catatonia manifested by catatonic symp
toms, fever, hemodynamic instability, and acute neurologic decline tha
t was associated with electrocardiographic and echocardiographic abnor
malities similar to those noted in patients with other central nervous
system processes. The patient's electrocardiographic and echocardiogr
aphic abnormalities resolved after successful electroconvulsive therap
y for the underlying neuropsychiatric disorder. The theoretic, physiol
ogic, and clinical significances of this case are discussed.
