We report a case of a patient with uterus didelphys, obliteration of t
he right cervical os, and septate vagina, with associated right hemato
pyocolpos and right renal agenesis. This 19-year-old woman with menarc
he at age 12 and regular periods every 28 days reported 2 years of non
cyclic, dull, nonradiating, right lower quadrant pain. This case is re
ported for two reasons. The first is to call attention to the usefulne
ss of radiologic modalities, such as ultrasound and MRI, in demonstrat
ing both external contour and internal arrangement of the organs. The
second reason is the interest of the case itself. The only finding tha
t prevented this from being associated with a right hematometra was th
e apparent closure at a point in the past of the right internal cervic
al os and retrograde menstruation through the fallopian tube on the ri
ght. Associated renal anomalies are discussed.
