There is a correlation between the degree of retinal changes and the s
everity of pregnancy induced hypertension. This correlation does not h
old true for a subgroup of these patients with hemolysis, elevated liv
er enzymes and a low platelet count. This array of findings has been t
ermed HELLP syndrome by Weinstein (1982) and may be present although t
he clinical picture of severe preeclampsia is absent. Pregnancies comp
licated by HELLP syndrome are associated with poor maternal and fetal
outcome. We report a case in which minor ocular signs led to the diagn
osis of a HELLP syndrome. Ophthalmologists should be aware of this rar
e complication of pregnancy.
