On initial presentation of a patient with IgD multiple myeloma there w
ere no features to suggest an unusual variant. Two months later she de
veloped spinal cord compression due to an IgD plasma-cytoma. This comp
lication of IgD has rarely been reported. During the course of the dis
ease and using the routine laboratory protocol for investigating and i
dentifying paraproteins, including IgD, the patient's results became i
ndistinguishable from those in Bence-Jones proteinuria myeloma.
