We describe the case of a patient in whom a syndrome of fever, pancyto
penia, pleural effusion, hepatosplenomegaly, positive ANA antibodies,
and bone marrow granulomas developed in association with tocainide the
rapy. Tocainide, a recognized, albeit rare, cause of fever, lupus-like
syndrome, and cytopenias, should be added to the list of medications
that can cause bone marrow granulomas.
