We present a case of a high-grade endometrial stromal sarcoma metastat
ic to the abdomen with an unusual extracellular hyaline matrix materia
l seen on fine needle aspiration biopsy, The patient was initially dia
gnosed with a stage IIIA high-grade endometrial stromal sar-coma and h
ad received four cycles of chemotherapy over the past year: She subseq
uently developed an abdominal mass that consisted of discohesive small
cells with scanty cytoplasm on fine needle aspiration. On the Diff-Qu
ik-stained smears, metachromatic, extracellular hyaline material was i
dentified. This appeared on the Papanicolaou-stained smear as cyanophi
lic material and did riot react with reticulin stain. This case emphas
izes the importance to the cytopathologist of including endometrial st
romal sarcoma in the differential diagnosis of hyaline matrix material
