A 37-year-old man with a history of chronic skin sarcoldosis had the c
lassic triad of lupus pernio. Each of the lesions (nose, ears, and han
ds) revealed avid radiogallium accumulation. In addition, both the pan
da sign and the lambda sign of hilar-mediastinal lymph node uptake of
radiogallium were present, despite normal results of chest radiography
. Several other lymph node chains were also involved. The case can ser
ve as a baseline to determine if other instances of lupus pernio have
such a wide systemic distribution of radiogallium uptake.
