Coronary arteries with anomalous aortic origin are infrequently encoun
tered in patients undergoing cardiac catheterization and pose potentia
l technical difficulties for performing both diagnostic and angioplast
y procedures. Percutaneous transluminal coronary angioplasty (PTCA) of
atherosclerotic disease in anomalous right coronary artery (RCA) has
been described in only a few reports. In all the cases reported to dat
e, the anomalous vessel arose from the left sinus of Valsalva. We repo
rt a case of a technically straightforward PTCA performed in an anomal
ous RCA arising from the anterior aspect of the ascending thoracic aor
