We describe a patient with POEMS or Crow-Fukase syndrome whose needle
renal biopsy specimen showed nephropathy, with findings compatible wit
h a mesangiocapillar glomerulopathy. This biopsy showed phenomena of g
lomerular mesangiolysis and tubulointerstitial damage with homogeneous
tubular casts. The immunofluorescence was positive for kappa light ch
ains, apparently in peritubular-linear form. We conclude that the rena
l pathology in POEMS syndrome seldom causes symptoms, but it may be mo
re frequent histologically. This nephropathy can show aspect s of glom
erular (mesangiolysis) and interstitial damage.
