The aim of the study was to elucidate the ability of high-resolution e
lectrocardiography to diagnose pathologic changes of atrial electrical
activity. Four groups of patients were studied: 90 patients with acut
e respiratory failure, 46 patients with attacks of paroxismal atrial f
ibrillation, 42 patients with rheumatic mitral valve disease, and 29 p
atients before and after coronary or valve surgery. It was shown that
analysis of the P-wave with the use of high-resolution electrocardiogr
aphy allowed to obtain important diagnostic information on the distrib
ution of exitation and electrophysiologic properties of atrial myocard
ium. Changes of values of power spectral density, low- and high-freque
ncy components, amplitude and temporal characteristics of frequency pe
aks were observed. It was found that antiarrythmic drugs affect some p
arameters of high-resolution ECG in patients subjected to mital valve
replacement or aorto-coronary bypass.
