The aim of the study was to assess value for prognosis of inhospital m
yocardial infarction and death of data obtained upon admission to coro
nary care unit of 94 patients with acute unstable angina. These patien
ts were included into double blind placebo controlled comparison of ef
ficacy of aspirin and intravenous heparin. At multifactorial stepwise
discriminant analysis the following 6 parameters were independently re
lated to death and development of myocardial infarction: long term use
of antihypertensive drugs before hospitalization, ST segment elevatio
ns on admission ECG, age, duration of unstable angina (in days), hepar
in use by the ambulance staff in prehospital treatment, white blood ce
ll count. In a subgroup of patients in whom parameters of hemostasis w
ere determined antithrombin III level entered the list of predictors o
f major cardiac events while levels of fibrinogen, protein C, activity
of tissue plasminogen activator inhibitor, cholesterol and lipoprotei
ns were not related to outcome of hospitalization.
