Sporotrichoid cutaneous leishmaniasis is due to dissemination of amast
igotes via the lymphatics to the subcutaneous tissues. A comparison wa
s made between the potential to disseminate by this route of 2 parasit
es of different zymodemes in Sudan and Saudi Arabia. In Sudan cutaneou
s leishmaniasis is caused by Leishmania major zymodeme LON-1, and in S
audi Arabia by L. major LON-4. Sporotrichoid leishmaniasis was signifi
cantly more common in Sudan, occurring in 23% of patients compared wit
h 10% in Saudi Arabia. Lymph node involvement was slightly more preval
ent in the Sudan. Clinical and pathological differences between subcut
aneous nodules, particularly when they ulcerate, and multiple primary
cutaneous lesions are described and treatment of localized and sporotr
ichoid leishmaniasis is discussed. The pathological features of the pr
imary lesions in the Sudan and Saudi Arabia were similar.
