A case is reported in which a patient with a rheumatoid factor-negativ
e rheumatoid arthritis developed rheumatoid nodules in the penis durin
g treatment with methotrexate. The development of rheumatoid nodules i
n seronegative rheumatoid arthritis patients is extremely rare. An acc
eleration of rheumatoid nodules in methotrexate-treated reheumatoid ar
thritis patients is reported in literature. Regarding the case reporte
d here, we propose a causal relationship between methotrexate-treatmen
t and the development of rheumatoid nodules in our patient. Methotrexa
te should not be the preferential treatment for patients with rheumato
id arthritis developing rheumatoid nodules and suffering from vasculit
is.
