Abstract
A 73-year-old man developed acute, painful, ophthalmoplegia. The pain improved with oral steroids and a diagnosis of Tolosa-Hunt syndrome wa s made. Review of his angiogram revealed a low flow dural arteriovenou s shunt that drained posteriorly. Dural arteriovenous shunts may thus be another cause of ''sinister'' Tolosa-Hunt syndrome.
