The detubularized right colonic segment is used as urinary reservoir i
n continent diversion and as bladder substitute. 21 patients underwent
continent urinary diversion and 22 male patients got a bladder substi
tute. In 4 of the patients with continent urinary diversion reoperatio
n was necessary (percutaneous nephrostomy, wound revision, revision of
the nipple). 4 of the 22 patients with bladder substitute had complic
ations, that is small-bowel obstruction, wound dehiscence and Candida
septicemia. Except for 2 men with bladder substitute all patients achi
eved continence. The use of the right colon as urinary reservoir does
not endanger the function of the upper urinary tract more than urinary
diversion by a conduit. The clinical results of these methods have be
en excellent.
