An ulcerated and eccentric distal right coronary artery plaque was fou
nd in a 56-year-old male with post-infarction angina. The 100 cm lengt
h of present DVI (Devices for Vascular Intervention, Inc., Redwood Cit
y, CA) atherectomy guiding catheters limits the ability to reach many
complex distal stenoses with the 125 cm Simpson Atherocath. After shor
tening the proximal portion of a standard DVI Judkins right guiding ca
theter without changing the distal contour, successful directional cor
onary atherectomy was performed.
