A 69-year-old man developed severe neck and back pain with paraparesis
which resolved within 2 h of onset. A CT myelogram demonstrated an ex
tensive anterior extradural haematoma. This and several other cases fr
om the literature suggest that patients with spontaneous spinal haemat
omas who rapidly recover from their neurological deficit do not requir
e urgent surgical decompression.
