Extension of myocardial infarction to the right ventricle usually pres
ents as congestive heart failure or cardiogenic shock, whereas neurolo
gical symptoms without shock are exceedingly rare. An unusual case in
which extension of the infarction to the right ventricle manifested as
status epilepticus without evidence of shock is reported. This case h
ighlights the clinical polymorphism of right ventricular infarction an
d suggests that the electrocardiogram should routinely include right p
recordial leads from V3R through V7R.
