Two cases of synovial sarcoma diagnosed by aspiration cytology are des
cribed. A 15-year-old, athletic female presented with a tender midthig
h mass clinically thought to be a hematoma. Aspiration cytology reveal
ed nu-merous clusters of spindle cells admixed with small groups of ep
ithelial cells. Cytokeratin and vimentin stains were positive in the e
pithelial and spindle components, respectively. Resection of the mass
revealed a bi-phasic synovial sarcoma. Second, a 53-year-old male pres
ented with a recurrent soft tissue mass 10 years after synovial sarcom
a of the knee had been resected. Aspiration cytology revealed neoplast
ic spindle cells. Cytokeratin and vimentin preparations showed focal p
ositivity. Resection revealed a predominantly monophasic synovial sarc
oma. These cases demonstrate the utility of aspiration cytology in dia
gnosing both unsuspected and recurrent synovial sarcoma.
