Although internal iliac artery aneurysms are uncommon, their timely re
cognition and surgical treatment are important since, like all other a
bdominal aneurysms, they tend to enlarge and rupture, often without wa
rning, and the outcome is usually fatal. An aneurysm of the left inter
nal iliac artery, in a seventy-six-year-old obese man, which ruptured
into the rectum, was successfully treated by partial removal, distal a
neurysmorrhaphy, and positioning of an aortofemoral prosthetic bypass.
