A case of hyperprostaglandin E syndrome, a variant of Bartter's syndro
me with prenatal onset, is described. Besides the history of polyhydra
mnios and prematurity, the infant showed the typical signs and symptom
s: hypokalemia, metabolic alkalosis, hyperreninemia, hyperaldosteronis
m, a normal blood pressure, hypercalciuria and hyperprostaglandin E-ur
ia, failure to thrive, and episodes of fever and vomiting. A literatur
e review of Bartter's syndrome revealed 44 similar cases of prenatal o
nset.
