We reported on a patient who had bilateral mesial frontal lesions, pre
sumably of a primary cerebral malignant lymphoma. The patient presente
d with a combination of unusual behavioral disorders of the upper extr
emities, which has not yet been documented in the literature: bilatera
l utilization and imitation behaviors and motor neglect of the left ar
m. Utilization behavior was closely associated with bilateral manual g
rasping behavior and was caused by the bifrontal lesions. Damage to th
e right supplementary motor area resulted most likely in concomitant m
otor neglect of the left hand. Discussion includes differentiation fro
m other related behavioral disorders.
