A 4-year-old child with traumatic rupture of the entire right hemi-dia
phragm and herniation of the liver into the chest is described. The in
jury is uncommon and the diagnosis by ultrasonography, fluoroscopy, ga
strointestinal contrast study, and CT scanning may be difficult or mis
leading. A non-ionic contrast, Omnipaque 240 (Iohexol) instilled via t
he intercostal catheter provides a definitive diagnosis rapidly and wi
thout complication.
