A 56-year-old man was scheduled for an orthopaedic operation on the sh
oulder to repair a rupture of the rotator cuff. Prior to surgery, he w
as diagnosed as having hypoproconvertinaemia associated with a factor
concentration of 10 %. Following the preoperative administration of 50
IU/kg of KG PPSB, severe disseminated intravascular coagulation (DIC)
developed, with subsequent multiorgan failure. For further operative
procedures that later became necessary to treat the sequelae of the DI
C, KG PPSB was withheld, replacement with fresh frozen plasma being ap
plied. A major reconstructive operation was carried out with a lower d
ose of 15 IU/kg KG PPSB in the form of another preparation with a high
er content of proconvertin being administered. Both the intraoperative
and postoperative phases were free of complications, and no further d
isseminated intravascular coagulation was observed The possible reason
s for the dramatic situation in this patient are discussed.
