A patient with hepatocellular carcinoma, associated segmental portal v
ein thrombosis, and accompanying pneumobilia, developed a liver absces
s and sepsis following transcatheter chemoembolization (TCE). It was b
elieved that the combination of bile duct necrosis after arterial occl
usion and pneumobilia led to ascending enteric infection and seeding o
f the necrotic tumor, which ultimately led to fatal outcome. We conclu
de that TCE is contraindicated in such cases.
