A patient presented with an acute right ventricular infarction charact
erized by an electrocardiographic current of injury in both the inferi
or (2,3,aVF) and anterior precordial leads (V1-V6). Cardiac catheteriz
ation demonstrated normal left ventricular wall motion, a codominant c
irculation, and severe disease of the right coronary artery. We propos
e that this coronary anatomy explains the injury currents on the elect
rocardiogram. This case illustrates a rare presentation of right ventr
icular myocardial infarction mimicking an extensive anterolateral wall
injury.
