The responses of renin, aldosterone, and blood pressure to ACTH suppre
ssion with dexamethasone in a 61-yr-old man with glucocorticoid-suppre
ssible hyperaldosteronism were modified by coexistent atheromatous ren
al artery stenosis (RAS). The apparent responsiveness of aldosterone t
o angiotensin-II resulting from RAS has implications for the regulatio
n of steroidogenesis in this condition. After successful surgical corr
ection of the RAS, the response changed and resembled that seen in two
younger males (one his son) with uncomplicated glucocorticoid-suppres
sible hyperaldosteronism.
