We report on a case of leprechaunism. In addition to the typical clini
cal and biochemical features, a bilateral juvenile granulosa cell tumo
r of the ovaries and cytomegalovirus hepatitis were found. The granulo
sa cell tumor may result from the mitogenic effect of insulin at high
concentration, which acts via a mechanism mediated by insulin-like gro
wth factor I receptors.
