The presence of intracoronary thrombus is associated with increased co
mplications during coronary angioplasty. Such thrombus may also mimic
the appearance of a critical stenosis. We report a case of nearly occl
usive intracoronary thrombus which resolved after prolonged heparin th
erapy, revealing only a minimal underlying stenosis. The recognition a
