Objectives. A new technique is described for primary realignment of pr
ostatomembranous urethral disruptions using endourologic techniques. M
ethods, Realignment was achieved in 3 patients on the day of injury. P
rimary urethral realignment is achieved by passing a council-tip cathe
ter over a guidewire inserted during antegrade flexible cystourethrosc
opy. Results. Three patients underwent successful realignment on the d
ay of injury. With an average follow-up of 6 months, 2 patients are vo
iding spontaneously without evidence of a urethral stricture; 1 patien
t developed a stricture necessitating delayed urethroplasty. All patie
nts have retained their potency and continence. Conclusions. Our techn
ique provides an expeditious, simple, and atraumatic means of obtainin
g primary realignment of acute prostatomembranous urethral disruptions
