Introduction: Tubular ectasia of the rete testis is a benign condition of c
ystic dilatation of the rete testis. A strong association exists between tu
bular ectasia and the presence of one or more ipsilateral spermatoceles. Ca
se Report: The case of a 49-year-old male with tubular ectasia is presented
and the typical sonographic appearance is described. Without any therapy t
he condition remained stable for two years of follow-up. Conclusions: In th
e case of doubtless sonographical diagnosis, missing palpable intratesticul
ar mass and ipsilateral spermatoceles the appearance of tubular ectasia sho
uld not be mistaken for malignancy. Further diagnostic investigations and s
urgical exploration of the testicle are, therefore, not necessary.
