The questions which a clinician tries to answer when faced with a smoker co
nsulting for help about giving up tobacco remain why an individual feels th
e need to consume the addictive product (tobacco) whose risks he knows and
to identify the psychological mechanisms underlying states of physical and
psychic dependence.
The main objective of the clinician consists, on one hand in showing how, s
tarting from a psychological evaluation, the characteristic features of a n
arcissistic disposition linked to tobacco addiction can be underlined and,
on the other hand, how the smoker's personality contributes to the explanat
ion of the degree and intensity of his tobacco addiction. The tobacco behav
iour or addiction may express a cluster of personality traits which open on
to the complexity of borderline states and various types of narcissistic s
uffering. These personality states could be associated with dependence beha
viour that might induce comportemental, physio-pathological and psyche path
ological disorders. An overview of relevant literature will throw up a ligh
t on what should be understood as tobacco addiction and narcissistic dispos
ition. We will see that the clinic of addictive behaviours constantly bring
s us back to serious and precocious narcissistic lacks, bringing about depr
essive experiences against which one should fight both in terms of behaviou
r and by means of the body.
The psychological evaluation such we use it allows us to circumscribe the p
ersonality and the addictive conduct of the smoker more accurately. It will
contribute to uncover the existing link between tobacco addiction and the
develop ment of discernible psyche pathological symptoms, to underline the
psyche pathological symptoms which seem to play a role in the persistence o
f tobacco addiction, to discern, once the addiction is well rooted, what pe
rtains of the previous personality of the subject and what corresponds to t
he modifications of his personality or behaviour under the effect of the to
xic. Moreover it palliates the difficulties of medical treatment by bringin
g precise therapeutic indications adapted to each smoker.
