The surgical repair of a ruptured thoracoabdominal aortic aneurysm in a 59-
year-old man with a transplanted kidney is reported. To reduce detrimental
sequelae to the kidney resulting from ischemia during simple crossclamping
of the aorta, perfusion of the renal allograft was ensured by using an aort
a-to-left-femoral-artery bypass with a centrifugal pump. The employed metho
d was found to be a safe and efficacious means of circulatory support to th
e renal transplant in the patient described here. Thirty-six reports (conce
rning seventy patients) of aortic repair in renal transplant recipients are
briefly reviewed with a discussion of management alternatives.
