Phacoemulsification in the presence of pseudoexfoliation of the lens p
resents surgeons with particular challenges. The frequency of intraope
rative and postoperative complications such as zonular dialysis, capsu
lar tears, vitreous loss, and intraocular lens (IOL) decentration may
be reduced with careful attention to detail and careful surgical techn
ique. Improvements in phacoemulsification technology, IOLs, and new ca
psular supporting rings allow surgeons to perform safer surgery in the
se eyes. We present the challenges of and options for managing catarac
t extraction in the presence of pseudoexfoliation.
