Surgery for highly placed basilar bifurcation aneurysms is one of the
most difficult operations in neurosurgery. Specific surgical technique
s have been developed including the temporopolar, zygomatic, transzygo
matic subtemporal, transclinoid trans-sellar transcavernous, and trans
third ventricular approaches. The authors present some technical adva
nces which have been developed for the transcristagalli translamina te
rminalis approach for the treatment of this aneurysm.
