The authors report a case of paradoxical embolism presenting with sync
ope and a transient cerebrovascular accident. A large thrombus was obs
erved entrapped in the foramen ovale during transthoracic echocardiogr
aphy and confirmed at transoesophageal echocardiography. Despite the r
ecent cerebrovascular event surgery was successfully performed. This c
linical situation, and a review of the literature illustrate the diagn
ostic value of transoesophageal echocardiography, the finding of an in
tra-atrial thrombus being a possible surgical indication when the clin
ical context is favourable.
