The case of a five-year-old girl with intractable fluor vaginalis and
recurrent perianal fistulas is reported. A presacral dermoid cyst was
shown to be the cause of these symptoms. Extensive radiological invest
igation revealed the cyst and the associated fistulas extending into t
he perianal region and the vagina. After verification of the anatomica
l situation, an uneventful surgical procedure was carried out. A relat
ively infrequent and heterogeneous group of tumors may arise in the pr
esacral space. Because of the vague nature of the symptoms, most of th
ese lesions are found only by careful examination. Recurrent perianal
fistulas should be a warning when routine surgery cannot resolve the p
roblems.
