We report an unusual case of prostatic carcinomatous meningitis and re
mind clinicians to maintain a high index of suspicion of meningeal inv
olvement when patients with advanced prostatic cancer present with cer
ebral symptoms, back pain, or neurologic findings. The diagnosis may r
equire repeated cytologic examinations of cerebrospinal fluid, and imm
unocytochemical stains should be considered to confirm a prostatic sou
rce if malignant cells are identified. Androgen ablative therapy may g
ive prolonged remissions, especially in patients with previously untre
ated tumours.
