Much of the published evidence regarding the use of biochemical marker
s in the acute coronary syndromes is contradictory, misleading, incomp
lete, and unsatisfactory. There are several problems relating to analy
zer calibration, reference ranges, decision thresholds, diagnostic gro
upings, practice guidelines, and diagnostic gold standards. Ln additio
n, much more rigor needs to be applied to test assessments, test use,
and economic and decision analysis.
