A case of scleredema diabeticorum in a patient with ''latent autoimmun
e diabetes in adults'' is presented. The skin biopsy material was stud
ied using histological, immunohistochemical and immunofluorescent tech
niques. There was swelling of the dermal collagen bundles with clear,
unstained spaces, or fenestrations, filled with serous materiel. Antin
uclear antibody revealed positive results for IgG 1:160 and IgM 1:80 (
both homogenous patterns). Anti-neutrophil-cytoplasmic-antibodies were
positive at 1:40 and islet cells antibodies (pancreatic) were positiv
e at 1:16. After the treatment (combination of insulin and metformin w
ith azathioprim) the clinical progression of SCLD gradually stopped, b
ut did not regress.
