Arteriovenous fistulas of the portal system are rare. Congenital fistu
las have to be differentiated from acquired fistulas; the latter are o
f posttraumatic or iatrogenic origin. The case presented demonstrates
a history of diffuse abdominal pain and the first description of myoca
rdial ischemia caused by a posttraumatic splenic arterioportal fistula
. After diagnostic and therapeutic difficulties, the definitive treatm
ent consisted in resection of the fistula system including the pancrea
tic tail.
