A 47-year-old woman with headache, a decline in general health, labora
tory evidence of inflammation, and multiple defects on radiographs of
the skull was thought at first to have either bone metastases or multi
ple myeloma. Histologic studies showed that the lesions were due to tu
berculous osteitis. Few cases of tuberculosis of the cranial vault hav
e been reported, The diagnosis depends on obtaining a biopsy specimen.
Appropriate therapy is followed by resolution of the symptoms and hea
ling of the radiologic lesions.
