A 69-year-old man with advanced prostate cancer was receiving antiandr
ogen therapy (bicalutamide [Casodex]). He developed dyspnea, periphera
l eosinophilia and bilateral pulmonary interstitial infiltrates. Trans
bronchial biopsy confirmed pulmonary eosinophilia. Withdrawal of bical
utamide and initiation of steriod therapy resulted in clinical improve
ment.
