Abstract
The traditional nursing process has changed over time, and contemporar y trends and forces suggest that another transformation of the nursing process is needed. The outcome-present state-test (OPT) reasoning mod el emphasizes reflection, outcome specification, and testing within th e context of clinical narratives. This transitional reasoning model bu ilds on the heritage of the nursing process and is more responsive and relevant to current nursing practice needs than previous nursing proc ess models.
