We report a case of spontaneous intracranial hypotension (SIH) that wa
s investigated using cranial MRI and radionuclide cisternography, Radi
onuclide imaging was remarkable, showing direct signs of diffuse asymm
etric leakage and indirect signs of cerebrospinal fluid (CSF) hypotens
ion consisting of slow CSF circulation to the convexity and rapid appe
arance of urinary bladder activity, The MRI appearance was also sugges
tive of SIH, with diffuse meningeal enhancement. Treatment with autolo
gous blood injection at the level of the radionuclide spinal leakage w
as useful, resulting in disappearance of SIH symptoms.
