We report a case of well differentiated perianal mucinous carcinoma wi
th associated dysplasia of the adjacent anal gland epithelium. Anal gl
and dysplasia is only rarely demonstrable histologically in Gases of a
denocarcinoma of suspected anal gland origin. The tumor was not associ
ated with chronic perianal abscess or fistula formation, which had bee
n regarded as important in the pathogenesis of perianal mucinous carci
noma. There was associated, clinically unsuspected Pegetoid intra-epid
ermal spread of adenocarcinoma in the perianal skin.
