Melkersson-Rosenthal syndrome is a rare condition which consists of pr
ogressive and recurrent orofacial edema, intermittent facial palsy, an
d lingua plicata. Labial swelling is the most common feature of this s
yndrome complex. The lips may enlarge up to three times their normal s
ize, resulting in aesthetic deformity and functional disability. Tradi
tional medical interventions are only marginally successful in treatin
g this syndrome. We present a case of Melkersson-Rosenthal syndrome wi
th massive lower lip enlargement, which was successfully treated by co
mbining a fleur-de-lis resection with a trans-modiolar labial suspensi
on suture. Five-year follow-up photographs are presented.
