We report a 49-year-old female with bilateral, painless, progressive l
oss of visual field due to retinitis pigmentosa sine pigmento. The pre
sence of optic atrophy and the lack of classic hone spicule changes in
this patient led to the erroneous diagnosis of a progressive optic ne
uropathy due to neurologic disease, Retinitis pigmentosa should be con
sidered in the differential diagnosis of visual field loss with or wit
hout optic atrophy even in patients lacking typical bone spicule chang
es in the retina, An electroretinogram may be the diagnostic study of
choice in these cases.
