Background. Polar and borderline lepromatosis leprosy can be complicat
ed by type 2 reactional states, including erythema nodosum leprosum. C
ase report. A 36-year-old man was treated for lepromatous leprosum. He
consulted for recurrent erythema nodosum leprosum. Certain nodular le
sions were bullous. Histopathology demonstrated major dermal edema cau
sing bullous blisters. Treatment with thalidomide led to rapid regress
ion. Discussion. The bullous form of erythema nodosum leprosum is rare
ly described in the literature. It raises the diffential diagnosis wit
h other bullous dermatoses, particulary Sweet's syndrome.
