The Community Re-Entry Program is a brief videotape-based skins traini
ng module designed to: (I) maize the patient an active collaborator wi
th the mental health system in fashioning an individualized aftercare
plan; and (2) equip the patient with the skills and knowledge needed t
o implement the plan and engage in longer-term, community treatments.
Results from two studies on the impact of the Community Re-Entry Progr
am point to its efficacy in linking patients with aftercare shortly af
ter discharge, but more research is needed to determine the durability
of aftercare engagement.
